An Introduction to Multiple Conditions for Housing Teams

Subject categories: General Health and Social Care, Housing

Course overview

This course has been developed to give learners an insight into a wide range of conditions and how each condition can affect an individual in different ways.  It is important that learners know how to begin to recognise these conditions as this may have an impact on their living arrangements if individuals do not gain support to be signposted to the appropriate services.
Within the Care Act 2014 it focuses on meeting individual’s needs and recognises that modern care and support can be provided in a number of ways. Guidance accompanying the Act, shows that when agencies involved in housing, health and social care work together they have more success than when acting alone.
This course is delivered using a range of methods and resources including:
A variety of condition fact sheets for research activity, Case studies, scenarios, group discussions and small group activities.
